Preparing for adulthood - support and services
Whilst moving into adulthood can be an exciting time for some young people, for others, and their families, it can be a worry as young people move on from one service and/or placement to another. Below is a list of local services and resources that can help families to navigate the next stage in their young person’s life.
The transition journey from age 14
This link provides information on transitions from children’s to adult services for education, health and social care and outlines the journey that a young person will take, dependent on the services involved. Young people and their families are involved in the transition process as much as possible to ensure that the care being delivered is appropriate for the young person’s individual needs.
Preparing for Adulthood Service Wandsworth
The Preparing for Adulthood Service is the social work (social care) team which supports young people with disabilities and mental health concerns from 14-25 years old and their parents/carers during the transition period from child to a young adult and includes leaving education, starting a job or work experience, state benefits and finances, moving into new accommodation etc.
